So You Want to Go to Prom?
Go below or to the prom tab to download, print, and fill out the student and guest forms for prom. Make sure you have ALL signatures, including a notary public. Even if you are 18, your parent/guardian is required to be present to certify your prom form. **If your guest is over 18, they do not need a notary signature**
You can go to your bank for a notary signature.
You no longer have to type on the form. Please print, fill out LEGIBLY, get all your signatures and have ready to turn in. If the form is ineligible we will make you redo it.
We’re selling April 5th 3-6PM and April 6th 11AM-2PM in the cafeteria.
You must have the student form filled out completely and the exact cash needed for one ($125) or two ($250) tickets.
WE ARE NOT SELLING EARLY AND SECURITY WILL BE PRESENT.
Only student forms are due when purchasing tickets; however, if you are planning on bringing a guest, you must pay for their ticket when you purchase yours. The prom guest form is due no later than May 15. If the form is not turned in by May 15th the prom guest will not be allowed to come to prom and there will be no refund. Additionally, there are NO REFUNDS* for students purchasing tickets to prom.
*this year we are offering a drawing for those students purchasing prom tickets on Day 1. If you purchase 1 or 2 prom tickets on Friday April 5th your name will automatically be put in a drawing to win 1 or 2 prom tickets. You will be notified of winning and your prom ticket cost will be refunded if you win. If we draw the name of a single ticket purchaser, another single ticket holder will be pulled which will account for the 2 tickets. If a ticket purchaser also pays for their date, their ticket will be paid for as well and this will account for the 2 free tickets.
